SPS Referral Forms

The referral forms below provide SPS community members an opportunity to share important information regarding SPS students directly with the SPS CARE Team & SPS Pre-College Team. These forms do not satisy mandatory reporting requirements - please see the Columbia University Report Forms below.

SPS Referral Forms Description
SPS CARE Team Referral Form

This referral form is to be used by community members of The School of Professional Studies(SPS) to make referrals to the SPS CARE Team. If you wish to speak with a CARE Team member please email sps-care [[at]] columbia [[dot]] edu (sps-care@columbia.edu)

SPS Pre‑College Referral Form

This form is to be used by School of Professional Studies staff members to document concerns, conversations, and other incidents that staff may encounter while working with the Pre-College program.

Columbia University Report Forms

The report forms below provide Columbia University community members an opportunity to report incidents and concernsdirectly with the appropriate University offices. 

Form Description
Academic Integrity Violation

Academic misconduct violates the principle of intellectual integrity, which is the foundation of our institution. Violating that principle is one of the most serious offenses a student can commit. Community members may use this form to report academic misconduct to the Center for Student Success and Intervention.

Gender Based Misconduct

This form should be used to refer an incident involving an alleged violation of:

  • Gender-Based Misconduct and Interim Title IX Policies and Procedures for Students  (including sexual harassment, sexual assault, gender-based harassment, domestic violence, dating violence, sexual exploitation, and stalking) by anyone wishing to file a referral on behalf of a Columbia University or Teachers College student. These allegations are investigated and adjudicated by the Gender-Based Misconduct Office;
  • EOAA Policies & Procedures  These allegations are investigated and adjudicated by Equal Opportunity and Affirmative Action.The University does not tolerate any form of gender-based misconduct, which includes sexual harassment, sexual assault, gender-based harassment, domestic violence, dating violence, sexual exploitation, and stalking. 
Discrimination, including antisemitism, anti‑Arab or anti‑Muslim discrimination

This form may be used to report an incident of discrimination or harassment. (Incidents involving faculty/staff are routed directly to the Office of Equal Opportunity and Affirmative Action . For gender-based incidents, please use the form above.) This form may be used to report discrimination or harassment incidents, including but not limited to the following:  

  • Age
  • Anti-Arab
  • Anti-Muslim
  • Antisemitism
  • Citizenship/Alienage
  • Disability
  • National Origin
  • Race/Color
  • Religion/Creed
General Concern Report

This report may be used to report violations of the Columbia University Rules of University Conduct, or other policies and to report general concerns involving Columbia University students and/or student groups, including bias.

Please ensure you select The School of Professional Studies (SPS) at the bottom of the form if submitting concerns for SPS community members.

These forms are available for SPS community members to provide feedback related to their academic and student experience. These are not official University report forms, please use the forms above to report concerns.

Course Evaluations

Course evaluations are one of the most important tools for instructors to monitor student feedback for teaching effectiveness. The School of Professional Studies administers end-of-term online course evaluations for all courses. The online evaluations are conducted through the course management system EvaluationKIT, which can be accessed via Canvas.

SPS Listening Owl

This form should be used to share feedback regarding your overall SPS experience. experience with our programs, services, and staff by completing the brief response form. We value questions and input from the members of our community and encourage you to provide us with your quality feedback. You may choose to be anonymous or you may provide us with a name and an email address for follow up from an administrator at the University.
